Candidate Review

Your Office: You begin by identifying if the patient is a good candidate for A04. You'll discuss with the patient that it may be too expensive to individually replace teeth and that they should get Dr. Calat's opinion.

You can instruct them to call either of our offices and schedule as a new patient for an A04 consultation.

NYNJOS: We'll meet and gather information with the patient during their first appointment. We'll perform a periodontal exam with necessary radiographs and cone beam scan.

Treatment Planning

NYNJOS: During the second visit, about one to two weeks later, our team presents the treatment plan, answers questions, discusses finances, and schedules your patient's surgery.

YOUR OFFICE: Meanwhile, you'll take impressions of both arches. You'll collect a deposit from the patient, and the lab will make the denture and send it to us.

Surgery Proceeds

NYNJOS: Our doctors perform the surgery, including extractions, implants, bone grafts, multi-unit abutments, and the loading of the immediate denture into a temporary bridge/hybrid.

We'll do a post-operative healing check a week later. If any issues arise, the patient can contact us.

Surgery Follow-Up

NYNJOS: We'll check implant health, photogrammetry and digital records (max/mand/bit/soft tissue) with a punch list sent to dentist and lab. Intraoral impressions can also be done at your office, if you prefer.

Your Office: You'll review the punch list (shade, midline, etc) and collect the patient's remaining balance so that the lab can make a digital provisional to replace the current provisional in the mouth.

The patient will return for the "try-in" teeth, and you'll need a 0.050 driver for the insertion. Most patients use this set of teeth for around two weeks to test comfort.

Final Fitting

Your Office: The patient will confirm if changes are needed. Occlusal adjustments normally require re-scanning; if not, notify the lab to go into production of the final teeth.You'll then do the insertion of the final zirconia hybrid - torque to 15Ncm.

NYNJOS: We will do a final observational visit one year later for updated radiographs and photos. All-on-4 treatment is complete!
